3/11/2024 EnchantaVaughn Approved 1.Behavior: Upon arrival to the home for todayâs session, Ms. Cosby reported that JaCaeri continues exhibit at-risk behaviors within the home and community. Ms. Cosby reported that JaCaeri continues to be disrespectful and talks back to authority figures within the home, school and community Ms. Cosby reported that had a good night last but woke up this angry for no reason. Ms. Cosby stated he told her âget the fuck out my roomâ at which these behaviors occur at least 4 times weekly with attitude lasting up to 20 minutes in duration. 2.Behavior: Ms. Cosby reported that JaCaeri got into a heated argument with his grandmother when she called for him to get up for to take out the trash and clean the home. Ms. Cosby reported that JaCaeri even called her out of her name when she asked him to stop talking to her with an disrespectful tone at which mother reports these behaviors are on a daily basis and can last up to 10 minutes in duration. 1.Intervention: QMHP-C continued to assessed and monitored JaCaeriâs behaviors towards authority figures and others. QMHP-C discussed with JaCaeri about his interactions are inappropriate and he needs to show respect to authority figures and others. QMHP-C informed JaCaeri that he cannot become aggressive and abusive when he feels things are not going his way. QMHP-C encouraged JaCaeri when he interacts authority figures and his peers to communicate instead of being forceful and aggressive. QMHP-C encouraged JaCaeri to use proper social skills to decrease aggressive behaviors. QMHP-C also encouraged JaCaeri to show respect for others and when something does not go his way he should try to compromise with others and continue to speak appropriate. QMHP-C allowed JaCaeri to write down and take notes during session. QMHP-C allowed family members to expressed utilizing effective communication techniques. 2.Intervention: QMHP-C continued to encouraged JaCaeri focus on techniques to assist him decreasing his behaviors. QMHP-C allowed JaCaeri to ask questions along with brainstorming ideas during todayâs session to assist him with understanding the effects of his behaviors within the home and community. QMHP-C discussed positive ways to assist JaCaeri with understanding how to follow directions without exhibiting reported behaviors within the home. QMHP-C modeled skills through role play for Ms. Cosby and JaCaeri. QMHP-C encouraged along with allowing Ms. Cosby, family members and JaCaeri to takes notes during todayâs session. QMHP-C provided Ms. Cosby and JaCaeri with contact information. 1.Response: JaCaeri went on to state that he did say some disrespectful things to his respectful things. JaCaeri reported that he was trying to stay calm but his mother made things worse when she started talking about his past behaviors. 2. Response: JaCaeri stated that she would find other outlets when upset. JaCaeri was very rude when speaking and expressing his feelings towards Ms. Cosby and his grandmother as they walked by towards the kitchen area...JaCaeri stated that he would also try to decrease his behaviors along with respecting his mother and grandmother,
